Abstract

OBJECTIVES: to give an overview of the use of safety devices in motorized vehicles in Italy in the years 2015-2017.
DESIGN: road side observational study.
SETTING AND PARTICIPANTS: a sample of 232,283 road users in 28 cities across the Italian territory with a resident population of more than 10 million inhabitants (17% of the Italian population) was considered.
MAIN OUTCOME MEASURES: the use of seat belts (front and rear), child restraints, and motorcycle helmets were monitored. For each city, the survey was carried out in urban, sub-urban, and extra-urban areas. Data were aggregated by three geographic areas: North, Centre, and South. Data for front seat belts was also provided for drivers and passengers separately.
RESULTS: the use of front seat belts shows a dramatic geographical trend ranging from 82.6% (North) to 36.3% (South). Drivers use seat belts more frequently than passengers (63.3% vs 57.4%). The same North-South trend shows the use of rear seat belts (from 19.0% to 3.3%) and child restraints (from 59.9% to 16.6%). Helmet use was high everywhere in Italy (more than 94%).
CONCLUSIONS: despite the use of safety devices, which has been mandatory for many years, compliance seems to depend on the voluntary behaviour of drivers and passengers influenced by socioeconomical and cultural patterns. The use of rear seat belts and child restraints is still far from an acceptable level, aswell as the use of front seat belts in the South and in the Centre. The increased use of safety devices would have significant public health and traffic safety implications, as a potential of 327 deaths/year could be saved.

OBIETTIVI: fornire una panoramica sull’uso dei dispositivi di sicurezza nei veicoli a motore in Italia negli anni 2015-2017.
DISEGNO
: studio osservazionale.
SETTING E PARTECIPANTI
: lo studio è stato condotto in 28 città del territorio nazionale con una popolazione residente di oltre 10 milioni di abitanti (pari al 17% della popolazione italiana), coinvolgendo un campione di 232.283 utenti della strada.
PRINCIPALI MISURE DI OUTCOME
: sono stati monitorati l’uso delle cinture di sicurezza (anteriori e posteriori), dei sistemi di ritenuta per bambini e dei caschi per motocicli. Per ogni città, l’indagine si è svolta in area urbana centrale, urbana periferica ed extraurbana. I dati sono stati aggregati in tre aree geografiche: Nord, Centro e Sud. I dati relativi alle cinture di sicurezza anteriori sono stati forniti separatamente per conducenti e passeggeri.
RISULTATI
: l’uso delle cinture di sicurezza anteriori mostra un accentuato trend geografico che va dall’82,6% (Nord) al 36,3% (Sud). I guidatori utilizzano le cinture di sicurezza più frequentemente rispetto ai passeggeri (63,3% vs 57,4%). Il medesimo trend Nord-Sud si riscontra per l’utilizzo delle cinture di sicurezza posteriori (dal 19,0% al 3,3%) e dei sistemi di ritenuta per bambini (dal 59,9% al 16,6%). L’uso dei caschi si è rivelato elevato ovunque in Italia (in media oltre il 94%).
CONCLUSIONI
: malgrado l’uso dei dispositivi di sicurezza sia obbligatorio da molti anni, il rispetto di tali prescrizioni normative appare essere condizionato anche dalle abitudini degli utenti della strada influenzate di modelli socioeconomici e culturali. L’uso delle cinture di sicurezza posteriori e dei sistemi di ritenuta per bambini è ancora lontano da un livello accettabile così come l’uso delle cinture di sicurezza anteriori nel Sud e nel Centro. Il maggiore uso di dispositivi di sicurezza avrebbe implicazioni significative per la salute pubblica e la sicurezza del traffico, poiché si potrebbe risparmiare un potenziale di 327 morti/anno.
