OBIETTIVI: valutare i fattori sociodemografici e comportamentali associati al sovrappeso e all’obesità nella popolazione immigrata in Italia.
DISEGNO: studio trasversale basato sul campione rappresentativo della popolazione straniera residente in Italia dell’indagine Istat “Condizione e integrazione sociale dei cittadini stranieri” 2011-2012.
SETTING E PARTECIPANTI
: lo studio è stato condotto sul sottocampione di 15.195 cittadini stranieri di età 18-64 anni, corrispondenti a una stima di 2.986.202 cittadini stranieri nella popolazione residente.
PRINCIPALI MISURE DI OUTCOME
: l’esito in studio è stato rappresentato dalle condizioni di sovrappeso o obesità, identificate secondo i criteri dell’OMS, rispettivamente per valori di indice di massa corporea (IMC) uguali o maggiori di 25 e 30 kg/m2. L’IMC è stato calcolato a partire dalle informazioni su peso (in kg) e altezza (in cm) raccolte dal questionario. Attraverso un modello multivariato log-binomiale è stata valutata l’associazione tra sovrappeso/obesità e i seguenti fattori: genere, età, area di provenienza, anni di permanenza, condizione occupazionale, famiglia mista, livello di istruzione, tipo di alimentazione, abitudine al fumo.
RISULTATI
: tra la popolazione straniera residente in Italia complessivamente il 30,9% è in sovrappeso (40,3% degli uomini e 23,2% delle donne) e il 7,5% è obesa (nessuna differenza per genere). Il modello log-binomiale multivariato evidenzia una probabilità di sovrappeso/obesità minore per le donne (PRR: 0,65) e per le famiglie in cui sono presenti italiani (PRR: 0,80); la probabilità cresce con l’età: 35-45 anni (PRR: 1,48), 46-55 (PRR: 1,73), 56-64 (PRR: 1,77) ed è maggiore per una permanenza in Italia di 5-10 anni (PRR: 1,11) o ≥11 anni (PRR: 1,09), per i non occupati (PRR: 1,05) e i meno istruiti (PRR: 1,06). Rispetto agli europei la probabilità è minore tra chi proviene dall’Africa sub-sahariana (PRR: 0,89), dall’Asia centro-occidentale (PRR: 0,93) e maggiore tra gli americani (PRR: 1,08).
CONCLUSIONE
: il problema del sovrappeso/obesità tra gli immigrati ha dimensioni analoghe a quelle dei cittadini italiani, il che rappresenta un fattore di preoccupazione anche perché il peso demografico delle seconde generazioni è ormai rilevante. Sono auspicabili interventi di sanità pubblica multifattoriali, cioè in grado di agire sia sulle abitudini alimentari sia sulla promozione dell’attività fisica, diffusi all’interno delle singole comunità attraverso messaggi culturalmente orientati.

Abstract

OBJECTIVES: to evaluate sociodemographic and behavioural factors associated with overweight and obesity among immigrant population in Italy.
DESIGN: cross-sectional study on the representative sample of foreign population resident in Italy of the Italian National Institute of Statistics (Istat) survey “Social conditions and integration of foreign citizens in Italy”, carried out in 2011-2012.
SETTING AND PARTICIPANTS: the study was conducted on a subsample of 15,195 foreigners aged 18-64 years, corresponding to an estimate of 2,986,202 foreign citizens among the resident population in Italy.
MAIN OUTCOME MEASURES: study outcome is the presence of overweight and obesity, conditions identified according to WHO criteria, namely body mass index (BMI) values ≥25 kg/m2 and ≥30 kg/m2, respectively. BMI was calculated based on weight (expressed in kg) and height (expressed in cm) information collected through the questionnaire. A multivariate log-binomial model was used to evaluate association between overweight/obesity and the following factors: gender, age, origin, length of stay in Italy, occupational status, presence of Italians in the household, educational level, dietary, smoking habits.
RESULTS: among foreigners in Italy, 30.9% is overweight (40.3% among men and 23.2% among women) and 7.5% is obese (no differences were found between males and females). The multivariate log-binomial model shows lower probability of being overweight/obese among women (PRR: 0.65) and among families with at least one Italian person (PRR: 0.80); probability increases with age: 35-45 years show a PRR of 1.48; 46-55 years a PRR of 1.73; 56-64 years a PRR of 1.77. The probability is higher if the length of stay in Italy is between 5 and 10 years (PRR: 1.11) or longer than 11 years (PRR: 1.09); it is higher also among not employed (PRR: 1.05) and less educated people (PRR: 1.06). If compared to European people, probability is lower among people who come from Sub-Saharan Africa (PRR: 0.89) and Central-Western Asia (PRR: 0.93), whereas it is higher among people from the Americas (PRR: 1.08).
CONCLUSIONS: overweight/obesity prevalence among immigrants has the same dimensions as for Italians. This is a worrying result, also considering the increasing demographic weight of second-generation immigrants. Multifactorial public health interventions in each community are advisable, both acting on dietary habits and on the promotion of physical activity, even trough culturally oriented messages.
