Riassunto

Obiettivi: valutare l’impatto sugli infortuni degli interventi di vigilanza in aziende del comparto costruzioni effettuati dal Servizio di prevenzione e sicurezza ambienti di lavoro (SPreSAL) di Novara.
Disegno: il disegno dello studio è del tipo “trattato-non trattato”, dove nei “trattati” sono comprese aziende oggetto di almeno un intervento di vigilanza dello SPreSAL nel triennio 2005-2007 e nessun intervento nel periodo precedente e successivo mentre i “non trattati” includono aziende con sede nel territorio dell’ASL di Novara senza nessun intervento di vigilanza dello SPreSAL nel periodo 2002-2010.
Setting e partecipanti: si sono utilizzate due fonti informative: i dati di vigilanza disponibili presso lo SPreSAL dell’ASL di Novara e i dati sulle aziende e sugli infortuni dei flussi informativi INAILRegioni. Il periodo di analisi è 2002-2010, suddiviso in sottoperiodi: pre-vigilanza: 2002-2004; vigilanza: 2005-2007; post-vigilanza: 2008-2010.
Principali misure di outcome: si è calcolato il tasso di infortuni (totali e gravi) e si è usato il metodo difference-in-differences (DID) per valutare l’efficacia degli interventi.
Risultati: il tasso di infortuni totali nel gruppo di intervento (trattati) è diminuito maggiormente da prima a dopo il periodo di intervento, rispetto al gruppo di controllo (DID: -1,95; IC95% -15,30;11,39). Per quanto riguarda gli infortuni gravi, invece, non si evidenzia alcun effetto (DID: -0,27; IC95% -7,84;7,31).
Conclusione: la vigilanza condotta nel 2005-2007 su aziende del comparto costruzioni operanti nel territorio dell’ASL di Novara ha avuto un piccolo impatto sulla riduzione del tasso di infortuni totali. Le procedure messe a punto in questo studio pilota possono essere utilizzate anche in altri Servizi PreSAL delle ASL italiane. Ulteriori approfondimenti sono necessari per confermare i risultati e per verificare i meccanismi che governano l’efficacia della vigilanza.

Abstract

Objectives: to describe a pilot study to evaluate the impact on injury rates of Occupational health and safety (OHS) inspections conducted in the construction sites of the Novara Health Unit territory during 2005-2007.
Design: study design is “treated/non treated” where: “treated” are construction companies inspected during 2005-2007 and not inspected in the periods before and after; “non treated” are construction companies never inspected during 2002-2010.
Setting and participants: two information sources have been used: Italian Workers’ Compensation Authority databases and Novara Health Unit inspection database. The period of analysis is 2002-2010 divided into sub-periods: pre-inspection (2002-2004); inspection (2005-2007); post-inspection (2008-2010).
Main outcome measures: two injury rates, total and serious, have been calculated and the difference-in-differences (DID) method was used to assess the effectiveness.
Results: passing from pre-inspection to post-inspection period, the rate of total injuries in the companies inspected decreased more than the rate of the companies not inspected (DID: -1,95; 95%CI -15,30; 11,39). No effect was shown on severe injuries (DID: -0,27; 95%CI -7,84;7,31).
Conclusion: the inspections conducted during 2005-2007 on the construction sites operating in the Novara Health Unit territory seem to have had a slight positive impact on reducing the injury rates. The procedures developed in this pilot study can be used in other Italian Health Units. Further research is needed to confirm the results and to clarify the mechanism by which the inspections were effective.
