OBIETTIVI: valutare l’incidenza del cancro della mammella (CM) in stadio avanzato – un indicatore surrogato precoce dell’efficacia dello screening mammografico – tra le donne che hanno aderito all’invito a partecipare al programma di screening della Regione Friuli Venezia Giulia (FVG) rispetto alle donne che non hanno aderito.
DISEGNO:
studio di coorte retrospettivo. Le donne invitate al primo round di screening (2006-2007) sono state identificate nell’archivio regionale del programma. La coorte è stata incrociata con l’archivio delle donne invitate al secondo round di screening (2008-2009). L’adesione è stata definita come adesione ad almeno uno dei due round. L’incidenza del CM al 31 dicembre 2013 è stata stabilita incrociando la coorte con il Registro tumori regionale. Sono state usate tre definizioni di stadio avanzato: pT2-4 (pT2+), linfonodi positivi (pN+), e stadio TNM II-IV (stadio II+).
SETTING E PARTECIPANTI:
programma di screening mammografico organizzato per le donne di 50-69 anni nelle 5 Aziende regionali per l’assistenza sanitaria.
PRINCIPALE MISURA DI OUTCOME:
rapporto tra tassi d’incidenza di CM (incidence rate ratio, IRR) aggiustato per età e indice di deprivazione, con intervalli di confidenza al 95% (IC95%).
RISULTATI:
la coorte comprendeva 104.488 donne aderenti e 49.839 donne non aderenti. Durante il follow-up (durata mediana 84 mesi), sono stati diagnosticati 2.717 CM tra le prime e 1.149 tra le seconde. L’incidenza totale del CM era più alta del 13% tra le aderenti (IRR 1,13; IC95% 1,05-1,21). Queste, tuttavia, avevano un tasso di CM pT2+ più basso del 36% (IRR 0,64; IC95% 0,56-0,72), un tasso di CM pN+ più basso del 13% (IRR 0,87; IC95% 0,78-0,98), un tasso di CM in stadio II+ più basso del 22% (IRR 0,78; IC95% 0,70-0,87), e un tasso di mastectomia più basso del 32% (IRR 0,68; IC95% 0,60-0,78).
CONCLUSIONI:
Le donne aderenti avevano tassi d’incidenza del CM in stadio avanzato significativamente più bassi. Questo effetto precoce anticipa una probabile riduzione della mortalità specifica.

Abstract

OBJECTIVES: to evaluate the incidence of advanced-stage breast cancer (BC) – an early surrogate indicator of effectiveness of mammography screening – among women who attended the Friuli Venezia Giulia (FVG) Region (Northern Italy) screening programme compared to women who did not attend.
DESIGN:
retrospective cohort study. Women invited to the first screening round (2006-2007) were identified from the database of the programme. The cohort was record-linked to the archive of women invited to the second round (2008-2009). The definition of attendance to screening was based on attendance to at least one of the two rounds. The incidence of BC was assessed through record linkage with the FVG cancer registry using an anonymous univocal identifier (end of follow-up: 31st December 2013). Three distinct definitions of advanced stage were used: pT2 or greater (pT2+), positive lymph nodes (pN+), and TNM stage II or greater (stage II+).
SETTING AND PARTICIPANTS:

MAIN OUTCOME MEASURES:
incidence rate ratio (IRR) between attenders and non-attenders, adjusted for age and deprivation index, with 95% confidence interval (95%CI).
RESULTS:
the cohort included 104,488 attenders and 49,839 non-attenders. During follow-up (median duration 84 months), 2,717 invasive BCs were diagnosed among attenders and 1,149 among non-attenders. Total incidence rate was 13% higher among attenders (IRR 1.13; 95%CI 1.05-1.21). CONCLUSIONS:
attenders had lower incidence rates of advanced-stage BC. This early effect is suggestive of a future impact of the screening programme on BC mortality.
